The Practice area includes Ormskirk, Aughton, Scarisbrick, Halsall, Haskayne, Bickerstaffe and some parts of Burscough and Lathom. A detailed map is available at Reception and also at the offices of Central Lancashire Primary Care Trust.

Registration

The easiest way to register is via this link https://gp-registration.nhs.uk/P81014

You can also call into the surgery if you wish, or fill in the Pre-Registration form below Newly registered patients will be invited for a check-up consultation if you have a chronic disease  within 6 months of registering. If you wish to choose to see a particular Doctor for some or all medical conditions please let the Receptionist know and it will be noted on your medical records. We will always try to ensure that this is possible but there may be occasions when it is not possible.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.
