Simply log in and select an option. <\/p>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\"> Online Appointments<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>We now offer a range of appointments that you can book online via patient access or any of the services listed below..<br>Wherever possible we try to offer a range of pre bookable appointments with our GP&#8217;s<\/p>\n\n\n\n<p>As of 1st July 2019 we will be offering same day urgent appointments which can be booked online. Please note that demand for same day appointments is high and so these appointments may not be available later in the day. You can even<a href=\"https:\/\/www.ormskirkmedicalpractice.nhs.uk\/clinics-and-services\/appointments-tests-referrals\/referral-for-further-care\/request-a-referral\/\"> self-refer<\/a> for some services without seeing your GP.<\/p>\n\n\n\n<div style=\"height:25px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>For&nbsp;<strong>real life-threatening emergencies<\/strong>&nbsp;such as those below \u2013&nbsp;<strong>RING 999<\/strong><\/p>\n\n\n\n<ul>\n<li><a href=\"https:\/\/www.nhs.uk\/conditions\/chest-pain\/\">Chest pain <\/a>(suspected heart attack)<\/li>\n\n\n\n<li><a href=\"https:\/\/www.nhs.uk\/conditions\/stroke\/symptoms\/\">Suspected stroke<\/a><\/li>\n\n\n\n<li><a href=\"https:\/\/www.nhs.uk\/conditions\/meningitis\/symptoms\/\">Suspected meningitis<\/a><\/li>\n\n\n\n<li>Anaphylactic shock (severe allergy)<\/li>\n\n\n\n<li>Heavy bleeding or deep lacerations<\/li>\n\n\n\n<li>Fluctuating levels of consciousness or completely unconscious<\/li>\n\n\n\n<li>Difficulty breathing or stopped breathing with a change in colour<\/li>\n\n\n\n<li>New seizure, fit or uncontrollable shaking<\/li>\n<\/ul>\n\n\n\n<p>For immediately&nbsp;<strong>serious conditions<\/strong>&nbsp;such as the following,&nbsp;<strong>GO TO Emergency Department (A&amp;E) IMMEDIATELY<\/strong><\/p>\n\n\n\n<ul>\n<li>A fever and lethargic (drowsy) child<\/li>\n\n\n\n<li>A feverish and floppy (unresponsive) infant<\/li>\n\n\n\n<li>Difficulty breathing<\/li>\n\n\n\n<li>Sudden, severe abdominal pain<\/li>\n\n\n\n<li>Accidental or intentional overdose of medication<\/li>\n\n\n\n<li>Trauma (including falls) and possible broken bones or road traffic accident<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"your-appointment-at-the-practice\">Your appointment at the Practice<\/h2>\n\n\n\n<ul>\n<li>Appointments can be made by telephoning the Surgery or calling in at Reception. We also offer the facility to book appointments online <\/li>\n\n\n\n<li>In line with The Department of Health requirements we offer &#8216;same day&#8217; appointments for urgent cases and pre-bookable appointments for routine matters. We&nbsp; pre-book up to two weeks&nbsp;in advance, with your GP<\/li>\n\n\n\n<li>Please make one appointment for each member of the family who needs to be seen<\/li>\n\n\n\n<li>Please be prompt on attending appointments. We try to keep to time but please be patient if someone before you takes longer than planned<\/li>\n\n\n\n<li>Appointments are generally for one or two&nbsp;problems therefore&nbsp;you may find it useful to book a double appointment if you have several problems to discuss to ensure that the doctor has enough time to be able to help you fully<\/li>\n\n\n\n<li>We also offer a Telephone Consultation Service with the GP as an alternative to attending the Surgery<\/li>\n\n\n\n<li>Appointments to see the Practice Nurses may be booked up to two weeks in advance<\/li>\n\n\n\n<li>It is Practice Policy to allow patients to choose whichever Doctor they wish to attend in the Practice.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"please-help-us\">Please help us<\/h2>\n\n\n\n<p>In the case of all appointments &#8211; particularly those that are pre-booked &#8211; if you no longer require the appointment we ask that you cancel the booking in order that the slot may be offered to a fellow patient thereby ensuring full use of valuable Practice time. If you are late for an appointment you may be asked to re-book.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/www.ormskirkmedicalpractice.nhs.uk\/clinics-and-services\/appointments-tests-referrals\/appointments-with-the-doctors\/cancel-an-appointment\/\">Cancel an appointment<\/a><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"additional-information\">Additional information <\/h2>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">Can I bring someone to accompany me to the Appointment?<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>We are generally very happy for patients to bring with them a carer, relative or even a friend. This often helps, particularly when they know you well and it allows them to tell us any observations they would make about you, which can help us in undertaking our assessment. Ultimately this will lead us more quickly to make an accurate diagnosis and therefore help you more.<\/p>\n\n\n\n<p>Also, having someone with you means they can prompt you to ask questions that you may have forgotten, and after the appointment they can help in reminding you what was discussed.<\/p>\n\n\n\n<p>Occasionally we might ask for them to leave but this would be unusual.<\/p>\n\n\n\n<p>If you require&nbsp;interpretation services please contact us in advance of you appointment and we will arrange this.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">If you have a suspected infectious disease<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>Please inform reception if you suspect an infectious disease, as this will enable us to deal with it appropriately during your visit to protect you, other patients and staff.&nbsp;<\/p>\n\n\n\n<p><\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">Giving Consent for Treatment<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>You have the right to accept or refuse treatment that is offered to you, and not to be given any physical examination or treatment unless you have given valid consent. If you do not have the capacity to do so, consent must be obtained from a person legally able to act on your behalf, or the treatment must be in your best interests.<\/p>\n\n\n\n<p>Your valid consent (agreement to the course of action) is needed for the treatment that\u2019s offered to you before any physical examinations or treatment can be given. If you haven\u2019t given your consent, you can accept or refuse treatment that\u2019s offered to you.<\/p>\n\n\n\n<p>It\u2019s important to be involved in decisions about your treatment and to be given information to help you choose the right treatment.
